What started as a small family-owned-and-operated clinic in the 1960s has grown into a state of the art healthcare facility while keeping the personal touch of our humble origin.

Our doctors are board certified in Family Medicine, a unique branch of medicine addressing the healthcare needs of your entire family. At CVFHC we have adopted the Osteopathic philosophy of complete patient care. This means that we will see you in a timely manner – same day if necessary, give you our undivided attention during your visit, and see you in follow up whenever appropriate.

We have plenty of free parking. We are located at 2800 SW 257th Ave, Troutdale, OR 97060 View on Mapquest Contact: 503-667-7711 We are open Monday through Friday from 8:00 AM – 5:00 PM. After hours calls not an emergency may be charged a $25 fee.

Payments
Payment is required at the time of service for all copays and non-covered services. We accept cash, checks*, American Express, Discover, VISA, and Mastercard.

Insurance Claims
Our business office will file a claim with your insurance company for services provided. Please verify with your insurance company prior to your visit that our doctors are listed providers on the insurance panel. Also, any services not covered by insurance will be the patient’s responsibility.

Pediatric Medicine (2+)

Preventive care including vaccinations, well-child exams, ear infections, pink eye, strep throat, allergies, fever.

Orthopedics

Evaluation and management of acute and chronic back pain, joint pain, and other musculoskeletal disorders.

Sports Medicine

Sports physicals prior to participation in all sports as well as sports-related injuries.

Worker’s Comp and Car Accidents

On-the-job or vehicular injuries sustained within 30 days of first visit.

Dr. Daisy Dhillon

Dr Dhillon graduated from the University of New England College of Osteopathic Medicine, Biddeford, ME, in 1996. She completed her internship and residency at Eastmoreland Hospital in Portland, OR, in 1999 and immediately joined our staff at Columbia View. Outside of medicine, Dr Dhillon enjoys spending time with her children and staying active

Dr. Matt Daskalos

Dr. Daskalos was born and raised in Oregon. He received his bachelor’s degree from Oregon State University, his medical degree from Western University of Health Sciences in Pomona, CA, and returned to Oregon for his residency training at Eastmoreland Hospital in Portland. Along with providing routine family care he has special interest in Orthopedic Injuries, Osteopathic Manipulation, Sleep Disorders and Preventive Healthcare.

Dr. Jennifer Byrne

Dr. Byrne received her bachelor’s degree from Yale University, New Haven, CT, and her medical degree from Oklahoma State University College of Osteopathic Medicine in Tulsa, OK. Her osteopathic internship was completed at Eastmoreland Hospital, Portland, OR, in 2004 and she received her residency training at Swedish Medical Center, Seattle, WA, in 2006. Dr. Byrne is interested in preventive medicine, pediatrics and women’s health.

Andrew Holden, FNP

Andrew completed his FNP program at the university of PENNSYLVANIA in philadelphia. He is board certified through the american academy of Nurse Practitioners. He has strong interest in endocrine and metabolic concerns, sleep disorders, allergies and immunology, cardiovascular disease, and mental health. Andrew practices evidence-based care. He believes each individual has unique needs that guide their healthcare and that modern and ADVANCED treatments should be available at the primary care level. he enjoys the great outdoors, trying new RESTAURANTS, and time with friends and family.

Dana Davies, FNP

Orignially from Wisconsin, Dana moved to Oregon in 2009 where she studied community health at PSU. She completed her nursing degree at Johns Hopkins, and returned to Portland to attend OHUS for her family nurse practitioner education. Dana hopes to create a fun, engaging, and comfortable primary care experience for her patients. She uses education and patient-centered care to empower patients in the management of their healthcare. In her free time, Dana enjoys spending time outdoors with friends and her dogs.

Skyler A. Stone, FNP

Skyler grew up in Western Washington and earned a Bachelor of Science in nursing from Washington State University in 2004 and a Master of Science in Nursing from Gonzaga University in 2009. Skyler has additional background experience in emergency medicine, upper extremity orthopedics and as an orthopedic surgical first assistant. He enjoys working closely with patients to understand and prevent a wide spectrum of chronic conditions. Acute orthopedic concerns of the shoulders and hands are also of particular interest to him. “Good health comes from education, prevention and overcoming physical and psychological barriers.” He spends his free time with his wife, son and two dogs coaching youth and high school soccer, skiing, biking and picking huckleberries. He is certified as a family nurse practitioner from the American Academy of Nurse Practitioners
